1. Application Letter (To be Scanned)

2. Ministry Approval if related to COVID-19 (To be Scanned)

3. New project application form (each page will be signed by all researchers participating in the study). (Signed form will be scanned - unsigned filled Word will also be sent)

4. Articles (3 copies). (PDF, Word)

5. Up-to-date CVs of all researchers to take part in the study, in the new format (Word)

6. Stamped and signed approval of the Head of the Related Department / Branches where the study will be conducted. (To be scanned)

7. Stamped and signed permission approval of the unit chief of the place where the research will be conducted (Chief Physician, Provincial Health Directorate, etc.). (To be scanned)

8. Good Clinical Practice Guidelines Commitment To be signed by all investigators participating in the study). (To be scanned)

9. Declaration of Helsinki (To be signed by all researchers participating in the study). (To be scanned)

10. Consent form for all studies except retrospective studies (including questionnaire) (PDF, Word)

11. If the study is retrospective, a letter of undertaking stating that patients' private information will not be used elsewhere and for other purposes. (To be scanned)
